Our Nurse Case Managers educate and train family members proper care and assistance for their loved ones diagnoses.

Patient advocates attend appointments, document diagnoses, prognosis, and directive care instructions. Following the appointments they educate and train their client’s loved ones in proper care, what to watch for, and what to do.
