Our Nurse Case Managers will provide you a detailed report of the previous months services and activities.
- Medical recommendations
- Complete list of providers along with all contact information
- Complete list of medications including name, dosage, purpose of medication, and prescribing physician
- Comprehensive medical update including a detailed list of activities performed by the nurse case manager in date order and activities recommended that indicates what the case manager intends to do over the next 30 days
